Saw an MF video that came fairly close, I think -- two dommes are taking turns forcing a dildo down a sub's throat until they get a little carried away by forcing it all the way in while simultaneously strangling her throat with a belt. They keep it up for a little too long, and the sub's eyes roll into the back of her head and she falls (I believe legitimately) unconscious. Wouldn't take much to nudge that into our world :)
